Our 90-Day Diabetes Management Program includes one-on-one virtual nutrition counseling with a registered dietitian, who provides customized meal strategies and lifestyle guidance. You'll also receive a complimentary personal training session with a certified fitness coach to help you create a personalized fitness plan.


To support your journey, you'll have access to our easy-to-use mobile app for tracking calories, meals, glucose levels, A1C, and progress. The program also features an interactive fitness webinar, led by our head personal trainer, that covers a variety of health and exercise topics.

Depending on your employer's wellness program you may qualify for prizes and rewards for your participation and/or progress in a wellness program. Prizes are at the discretion of your employer and only provided by your employer, not Health Fitness Connections. Ask your HR department if our program qualifies for any company incentive(s).  

NUTRITION COUNSELING SESSION OUTLINE FOR Diabetes Management

Our diabetes management program focuses on education, behavior change, and personalized support to empower individuals to make healthier dietary choices. Our goal is to help you reduce your risk of further complication from effects of diabetes through one-on-one nutrition counseling with our registered dietitian. 


Below is an outline of topics you may discuss with our registered dietitian during your one-on-one nutrition counseling sessions. Topics covered may vary from client to client and the outline below is an example and should not be used as an exactly schedule.  Our registered dietitian and you will ultimately plan an agenda that works best for you. 


Session 1: Introduction to Diabetes Program

  • Goals: Understand diabetes risk factors and the role of nutrition.
  • Topics Covered:
    • Overview of diabetes (types, risk factors, statistics)
    • Importance of healthy eating and lifestyle changes
    • Setting personalized goals for prevention
  • Activity: Initial assessment (dietary history, health metrics)


Session 2:  Understanding Carbohydrates

  • Goals: Learn about carbohydrates and their impact on blood sugar.
  • Topics Covered:
    • Types of carbohydrates
    • Insulin resistance
    • Counting carbs and pairing protein
    • Managing blood sugar throughout your day.
  • Activity: Carbohydrate counting exercise using food labels


Session 3:  Building Balanced Meals

  • Goals: Create balanced meals to stabilize blood sugar levels.
  • Topics Covered:
    • Components of a balanced plate (proteins, fats, fiber)
    • Importance of portion control and meal timing
    • Ideas for healthy snacks and meal prep
  • Activity: Develop a weekly meal plan together


Session 4:  Incorporating Fruits and Vegetables

  • Goals: Increase the intake of fruits and vegetables.
  • Topics Covered:
    • Benefits of fiber and phytonutrients for blood sugar control
    • Strategies to include a variety of produce in meals
    • Understanding serving sizes for fruits and vegetables
  • Activity: Create a list of favorite fruits and vegetables and plan how to incorporate them


Session 5:  Mindful Eating and Lifestyle Factors

  • Goals: Develop mindful eating habits and understand lifestyle influences.
  • Topics Covered:
    • Techniques for mindful eating (e.g., paying attention to hunger cues)
    • The impact of stress, sleep, and physical activity on diabetes risk
    • Strategies for overcoming emotional eating
  • Activity: Mindful eating practice during a meal or snack


Session 6  Review and Long-Term Planning

  • Goals: Reflect on progress and create a sustainable plan for prevention.
  • Topics Covered:
    • Recap key concepts learned throughout the program
    • Celebrate successes and identify areas for continued improvement
    • Develop a long-term action plan for maintaining healthy habits
  • Activity: Goal-setting worksheet for future health management


Frequently Asked Questions

There is no cost for any of the services in our program. However, in rare cases, your health insurance plan may impose a cost for nutrition counseling. 


Yes. You must: 

1) have a diabetic diagnosed as primary care physician.

2) have health insurance that covers nutrition counseling/medical nutrition therapy. In most cases they will cover nutrition counseling at 100%.

Keep in mind you should contact your insurance plan for eligibility if you are unsure if your plan covers nutrition counseling or you can complete our secure  insurance verification form


The fitness webinar is designed to help you understand the foundations of fitness and how to exercise effectively—no matter your age, ability, or experience. Each topic focuses on actionable tips you can start using immediately. You will also have the opportunity to ask our fitness professional fitness related questions and receive live responses.

As part of our commitment to your health and wellness, we’re offering a free personal training session with one of our certified fitness professionals. Sessions may be in-person or virtual.  


During this one-on-one session, you’ll receive:

  • Goal-focused workout guidance
  • Expert tips on form, technique, and progression
  • Answers to your fitness questions and concerns

We also offer partner training (you, a workout buddy, and our personal trainer) and small group training (you, and up to 4 other workout buddies, and our personal trainer).


A debit/credit card on file will be required prior to booking your appointment.


To ensure fairness and respect for our trainers' time, we kindly ask all clients to adhere to the following cancellation policy: 

  • 24-Hour Notice Required: Cancellations or rescheduling must be made at least 24 hours in advance of your scheduled session.
  • Late Cancellations: There will be a $20 charge to your card for sessions canceled less than 24 hours' notice.
  • No-Shows: There will be a $30 charge if you fail to attend a scheduled session without notice.
  • Trainer Cancellations: If your trainer needs to cancel, we will make every effort to reschedule your session at your convenience.
